This Hospital Intends to purchase the under mentioned item.

Medicine
Medical Consumable
Gen Store

The terms and conditions of the tender are as under:-
a. Rates must be valid for one year, FROM THE DATE OF OPENING of the tender.
b. Quoted rate for Medical Consumable and Gen Store will be reviewed after every quarter.
c. The purchaser retains the authority to REJECT any or all the quotations without assigning any reason.
d. Rates quoted for Stores/Equipment must be as per SPECIFICATIONS/ Sample shown.
e. Tender Documents can be obtained on 12 Dec 2011 between 09:00-12:00 hrs.
f. Quotations must be dropped in the tender box placed in GM Logistics Office between 15 to 21 Dec 2011.
g. The tender will be opened on 22 Dec 2011 at 11:00hrs.
h. Presence of the Firms/their representative at the time of opening is Mandatory.
i. INABILITY may be intimated and Tender Document be returned with the remarks “NOT QUOTED”.
j. Make, Model and country of origin of the item must be specified.
k. More then one make/model may also be quoted giving separate price for each make/made.
l. All the taxes i.e. INCOME Tax AND OTHER CHARGES should be included in the quoted rates.
m. Firms must be registered with Income Tax & Sale Tax authority. Copy of NIC, NTN and GST is attached with Tender documents.
n. Firms offering rates on CFR must attach letter of authorization/letter from Principal/proprietary Certificate from Principal.

In case of exemption of taxes firm must attaché copy of letter issue by concern authorities.
